OVERVIEW

Medication reconciliation is a requirement of the CoPs, but how do we reconcile all the OASIS items that include medications? Fourteen OASIS-E items, including six new items, require staff to take action on medications. These items have different time points, timeframes, and define medications differently depending on the item.
