In the world of home health care, preventing unnecessary hospitalization is a top priority. One of the most critical tools clinicians use to assess this risk is the M1033 OASIS item, titled “Risk for Hospitalization.” Updated in October 2023, this assessment item plays a pivotal role in identifying patients who may be at higher risk of acute care episodes, allowing for timely interventions and improved outcomes. Results of the assessment and screening can be used to comply with a standard of the Conditions of Participation requiring identification of risk factors, interventions, and goals to mitigate those factors.

What is M1033?

M1033 is part of the Outcome and Assessment Information Set (OASIS) data collection used in home health care. It evaluates a patient’s risk for hospitalization based on a variety of clinical and behavioral indicators. The more risk factors present, the higher the likelihood of hospitalization, which can impact both patient outcomes and agency performance under the Patient-Driven Groupings Model (PDGM).

Key Risk Factors Assessed in M1033

The M1033 item includes a checklist of 10 risk factors, categorized by different look-back periods:

12-Month Look-Back

  1. Falls – Two or more falls, or any fall with injury.
  2. Weight Loss – Unintentional loss of 10 pounds or more.

6-Month Look-Back

  1. Multiple Hospitalizations – Two or more acute care hospitalizations.
  2. Multiple Emergency Department (ED) Visits – Two or more ED visits, excluding urgent care or physician office visits.

3-Month Look-Back

  1. Mental, Emotional, or Behavioral Decline – Any noted decline, temporary or permanent.
  2. Difficulty Complying with Medical Instructions – Includes medication noncompliance, missed appointments, or lack of necessary equipment.

Day of Assessment

  1. Taking Five or More Medications – Includes all prescribed and over-the-counter medications, supplements, and oxygen.

Other Considerations (No PDGM Points)

  1. Reports of Exhaustion
  2. Other Risks Not Listed – Such as dialysis, terminal illness, low literacy, or unsafe environments.
  3. None of the Above – Should be used sparingly.

The Functional Scoring Impact of M1033

Checking four or more of the first seven options on M1033 results in 12 functional points. These points are used to categorize patients into low, medium, or high functional impairment levels, which directly influence reimbursement under PDGM. Because of this, accurate and thorough documentation is essential to support the selections made on M1033.

M1033’s Role in Value-Based Purchasing (VBP)

Beyond PDGM, M1033 plays a significant role in Value-Based Purchasing (VBP):

  • Discharge Function, which accounts for 20% of the Total Performance Score (TPS), uses M1033 as a risk variable to adjust outcomes fairly across patient populations.
  • M1033 also indirectly influences the Potentially Preventable Hospitalization metric, which makes up 26% of the TPS, through its impact on the functional score category.

Conclusion: Why M1033 Matters More Than Ever

M1033 is more than just a checklist—it’s a strategic tool that bridges clinical insight with performance-based outcomes. By accurately identifying risk factors for hospitalization, clinicians can tailor care plans, prevent avoidable hospital visits, and ensure patients receive the support they need at home. At the same time, M1033 influences key metrics that affect agency reimbursement and quality ratings under PDGM and VBP.

In short, M1033 is a linchpin in both patient care and agency success. Ensuring accurate, well-documented responses not only improves patient outcomes but also strengthens your agency’s performance in an increasingly data-driven healthcare environment.
