IRF: Strengthening Your IRF Documentation

Improved documentation practices will help in defending your IRF medical records against audit scrutiny especially with a looming Review Choice Demonstration (RCD).

This workshop focuses on the enhancements IRFs need to make to their documentation, from preadmission to discharge, in order to accurately support CMS reasonable and necessary criteria, comply with CMS’ documentation requirements and complete the most recent IRF-PAI.

The presenters will facilitate discussion and action planning in order to examine documentation practices and identify opportunities for improvement. Special attention will be given to ensuring that the documentation in each medical record clearly supports CMS criteria and the data recorded on the IRF-PAI, particularly the quality indicator items.
